Apply for Birth certificate

In Application of Birth Certificates, there are two categories 

1. Birth Certificate - registration of births that happened less than a year ago.
2. Late Registration of Birth

Birth Certificate

Citizen can apply for the certificate at their particular Municipality/Panchayat Office by providing the Doctors Certificate given by the recognized authorities. The service is for registration of births that happened less than a year ago.

Late Registration of Birth

For late registration of births ie. births that happened more than a year ago, citizens can apply for birth certificate through nearest Meeseva center. 

Required Documents:

1. Physical Document
2. Non availability issued by Gram Panchayat / Municipal Administration
3. Ration Card Copy
4. SSC Marks Memo
5. Self Affidavit

Application for late regsitration of birth

After Getting LRBD Certificate from Revenue Department, the applicant can go to Municipality/ Gram Panchayat to collect the Birth Certificate. 

Corrections in Birth certificate

Visit your nearest Meeseva centre to apply for corrections in Birth certificate. 

Application for changes in birth certificate

Documents required

1. Declaration by the father / mother in case of live Birth. The declaration stated should be true and correct by two Gazetted Officers (the name of officers to be written in Capitals)
2. Notary affidavit on (Rs. 10/. Non judicial Stamped paper)
3. The original Birth certificates already taken are to be returned.
4. Documentary evidences like educational certificates , election ID card, Ration card, passport, Driving Licenses, Marriage certificate, LIC policies, caste certificates, etc)
5. Consent letter from concerned Hospital regarding the correction to the effect.
6. Other child certificates if any.
