Unique Disability ID

"Unique ID for Persons with Disabilities” project is an initiative of Department of Empowerment of Persons with Disabilities, Government of India. It aims at building a holistic end-to-end integrated system for Issuance of Universal ID & Disability Certificates for Person with Disabilities with their identification and disability details.

The UDID card has a total of 18 digits/characters. The first 2 characters represent State Code, next 2 digits for district code, next 1 digit for CMO code, next 2 digits for disability type, next 4 digits for year of birth of PwD, following 6 digits for running number and the last digit for check sum which is involved for security reasons.

About UDID

The UDID project is being implemented with a view of creating a National Database for PwDs, and to issue a Unique Disability Identity Card (UDID) to each person with disabilities.

It includes -

  • Online availability of data of Person with Disabilities across country through a centralized web application
  • Online filing and submission of registration application form for disability certificate/ Universal ID card; Offline applications may also be accepted and subsequently digitized by agencies
  • Quick Assessment process for calculating the percentage of disability by the hospitals/ Medical Board
  • Non-duplication of PwDs data
  • Online renewal and update of information by Person with Disabilities/ on their behalf
  • MIS reporting framework
  • Effective management including interoperability of the benefits / schemes launched by the Government for PwD
  • To take care of additional disabilities in future. Number of disabilities at the moment isseven and shall be subject to increase as per the new Act/ notification which can be up to 19 or more.

As per RPWD Act, 2016, UDID card can be issued by home district hospital as well as the hospital where the PWD is taking medical treatment.

Benefits of UDID

The UDID card shall bring a host of benefits to the Persons with Disabilities as given below:

  1. Persons with disabilities will not need to make multiple copies of documents, maintain, and carry multiple documents as the card will capture all the necessary details which can be decoded with the help of a reader
  2. The UDID card will be the single document of identification, verification of the disabled for availing various benefits in future
  3. The UDID Card will also help in stream-lining the tracking of the physical and financial progress of beneficiary at all levels of hierarchy of implementation – from village level, block level, District level , State level and National level.

When disability is less than 40 percent, the use of UDID card is as follows

As per notification issued by DEPwD on 16th December, 2022-

  • All  our National Institutes (NIs)  and  CRCs,  Registration/Diagnostic  Treatment  Fee  may  be waived off for UDID card holders, as well as for those who have disability certificate and have got enrolled on UDID portal, irrespective of percentage (%) of disability, w.e.f. 1st January, 2023.
  • Full course fee may be waived off for those students of various courses at our NIs and CRCs, who are UDID card holders, as well as for those who have disability certificate and have got enrolled on UDID portal, irrespective of percentage (%) of disability. This should be done for 2022-23 batch students onwards (including those who are doing two year or more courses and are in their 2nd/3rd/4th year.

Types of UDID Card

Three types of coloured based UDID Cards are issued based on the severity of the disability-

  • White Card: When the disability percentage of a PwD is below 40%
  • Yellow Card: When the disability percentage of a PwD is above 40% but on or below 80%
  • Blue Card: When the disability percentage of a PwD is above 80%

Application process

Who can apply?

Any citizen of India with full/partial disability can apply for UDID card.

List of Disabilities:

  • Physical disability 
    • Locomotor Disability (a person's inability to execute distinctive activities associated with movement of self and objects resulting from affliction of musculoskeletal or nervous system or both), including
      • "leprosy cured person" means a person who has been cured of leprosy but is suffering from
        • loss of sensation in hands or feet as well as loss of sensation and paresis in the eye and eye-lid but with no manifest deformity;
        • manifest deformity and paresis but having sufficient mobility in their hands and feet to enable them to engage in normal economic activity;
        • extreme physical deformity as well as advanced age which prevents him/her from undertaking any gainful occupation, and the expression "leprosy cured" shall construed accordingly;
      • "cerebral palsy" means a Group of non-progressive neurological condition affecting body movements and muscle coordination, caused by damage to one or more specific areas of the brain, usually occurring before, during or shortly after birth;
      • "dwarfism" means a medical or genetic condition resulting in an adult height of 4 feet 10 inches (147 centimetres) or less;
      • "muscular dystrophy" means a group of hereditary genetic muscle disease that weakens the muscles that move the human body and persons with multiple dystrophy have incorrect and missing information in their genes, which prevents them from making the proteins they need for healthy muscles. It is characterised by progressive skeletal muscle weakness, defects in muscle proteins, and the death of muscle cells and tissue;
      • "acid attack victims" means a person disfigured due to violent assaults by throwing of acid or similar corrosive substance.
  • Visual impairment
    • "blindness" means a condition where a person has any of the following conditions, after best correction
      • total absence of sight; or
      • visual acuity less than 3/60 or less than 10/200 (Snellen) in the better eye with best possible correction; or
      • limitation of the field of vision subtending an angle of less than 10 degree.
    • "low-vision" means a condition where a person has any of the following conditions, namely
      • visual acuity not exceeding 6/18 or less than 20/60 upto 3/60 or upto 10/200 (Snellen) in the better eye with best possible corrections;
      • limitation of the field of vision subtending an angle of less than 40 degree up to 10 degree.
  • Hearing impairment
    • "deaf" means persons having 70 DB hearing loss in speech frequencies in both ears;
    • "hard of hearing" means person having 60 DB to 70 DB hearing loss in speech frequencies in both ears;
  • "Speech and language disability" means a permanent disability arising out of conditions such as laryngectomy or aphasia affecting one or more components of speech and language due to organic or neurological causes.
  • Intellectual disability, a condition characterised by significant limitation both in intellectual functioning (rasoning, learning, problem solving) and in adaptive behaviour which covers a range of every day, social and practical skills, including
    • "specific learning disabilities" means a heterogeneous group of conditions wherein there is a deficit in processing language, spoken or written, that may manifest itself as a difficulty to comprehend, speak, read, write, spell, or to do mathematical calculations and includes such conditions as perceptual disabilities, dyslexia, dysgraphia, dyscalculia, dyspraxia and developmental aphasia;
      • "autism spectrum disorder" means a neuro-developmental condition typically appearing in the first three years of life that significantly affects a person's ability to communicate, understand relationships and relate to others, and is frequently associated with unusal or stereotypical rituals or behaviours.
    • Mental behaviour- "mental illness" means a substantial disorder of thinking, mood, perception, orientation or memory that grossly impairs judgment, behaviour, capacity to recognise reality or ability to meet the ordinary demands of life, but does not include retardation which is a conditon of arrested or incomplete development of mind of a person, specially characterised by subnormality of intelligence.
    • Disability caused due to
      • chronic neurological conditions, such as
      • "multiple sclerosis" means an inflammatory, nervous system disease in which the myelin sheaths around the axons of nerve cells of the brain and spinal cord are damaged, leading to demyelination and affecting the ability of nerve cells in the brain and spinal cord to communicate with each other;
    • "parkinson's disease" means a progressive disease of the nervous system marked by tremor, muscular rigidity, and slow, imprecise movement, chiefly affecting middle-aged and elderly people associated with degeneration of the basal ganglia of the brain and a deficiency of the neurotransmitter dopamine.
  • Blood disorder
    • "haemophilia" means an inheritable disease, usually affecting only male but transmitted by women to their male children, characterised by loss or impairment of the normal clotting ability of blood so that a minor would may result in fatal bleeding;
    • "thalassemia" means a group of inherited disorders characterised by reduced or absent amounts of haemoglobin.
    • "sickle cell disease" means a hemolytic disorder characterised by chronic anemia, painful events, and various complications due to associated tissued and organ damage; "hemolytic" refers to the destruction of the cell membrane of red blood cells resulting in the release of hemoglobin.
    • Multiple Disabilities (more than one of the above specified disabilities) including deaf blindness which means a condition in which a person may have combination of hearing and visual impairments causing severe communication, developmental, and educational problems.
  • Any other category as may be notified by the Central Government

How to apply?

For Fresh Cases:

Click Apply online, fill online application, attach scanned copy of required documents.

  1. Person with Disability already having Disability Certificate ( Whose data have been migrated in UDID Portal) : All PwDs who have been issued Disability Certificate  visit https://www.swavlambancard.gov.in/pwd/searchapplication.  To get your details search by Beneficiary ID/State ID  or Aadhaar Number (if linked). On receipt of your details, fill up the application and submit.
  2. Person with Disability already having Disability Certificate (Whose data have not been migrated in UDID Portal): Click Apply online, fill up application as fresh and select option “Have Disability Certificate?: Yes” in Disability Details Tab . Fill up other details and submit application.
  3. Person with Disability not having Disability Certificate: Click Apply online, Choose option “Have Disability Certificate? No” in Disability Details Tab while filling up application and submit application.

Four types of details are required:

  1. Personal Details including Address
  2. Disability Details
  3. Employment Details
  4. Identity Details

What is the validity of UDID Card?

The UDID card has two types of validity- Permanent UDID card and Temporary UDID Card.

  • Permanent UDID: The competent medical authority issues Permanent UDID Card considering there is no progress or regress in the disability of a person
  • Temporary UDID : The competent medical authority issues Temporary UDID Card considering there is a progress or a regress in the disability of a person
